First call does not work with Bluestar

Just installed Bluestar today in a 2008 GMC Yukon Denali. I am using it with an iPhone 4 (iOS 4.3.3) and the Bluestar is running 11-0401 firmware.
Everything works great except for the very first call I place or receive after getting into my car does not have audio over the car's speakers. Let me describe what happens:

Scenario 1:
Turn on Car, Bluestar shows paired with phone
Press white button to voice dial - phone goes into voice dial mode but the radio does not mute (music is still playing)
Press white button to cancel voice dial
Press white button to voice dial again, this time the radio mutes and the voice dial feature is heard over the car speakers

Scenario 2:
Turn on Car, Bluestar shows paired with phone
Phone starts ringing with inbound call
DIC shows inbound call and the telephone number
ringing is not heard over the speakers, and the radio does not mute
I can answer the call by pressing the white button, but the call audio is not heard over the speakers
second inbound call works fine

Scenario 3:
Turn on Car, Bluestar shows paired with phone
Place outbound call from Phone
DIC shows On Call
No phone audio is heard over speakers, radio does not mute
Second outbound Call works fine

Basically it seems like the very first inbound or outbound call does not work over the radio. After that however, the phone works flawlessly with Bluestar.

Any ideas as to what could be causing this problem?

Similar issue with me as well, first incoming call rings, but I can not hear caller, but they can hear me. Eventually I hang up and they re-call and it works perfectly.

Issue with coastal lock pick gmx322 found to cause this.
Newer versions of lock pick seem to be ok.
